Before you proceed, please print this page of instructions for reference as you go through the process of completing an adoption information form.

To begin, click on the Add link below.

The database information form will appear.

Complete the form.

At the bottom of the adoption information form click on the "add record" button. . If you receive error messages, simply correct the errors and click on the "add record" button again. When the form appears (error free) it should have all the information you entered. Please follow the next steps carefully:

You should click on "log off" at the bottom of the page. This is important! If you exit the add screen any other way, the record will not be added correctly.

When you click on "log off" you will get a verification screen.

You should enter a valid e-mail address can click on "submit".

You will receive an e-mail to confirm your addition to the database. When you receive your e-mail, please save it for future reference.
